Vital Records

Birth Certificates

Certified copies of birth certificates can be obtained from the Town Clerk’s Office.  Birth certificates are on file for births which occurred in Goshen, or for children whose parent(s) were residing in Goshen at the time of the birth.

The fee for a birth certificate is $5 per certified copy (cash or check made out to the Town of Goshen), and can be obtained by mail or in person.  Please include the full birth name, date of birth and name of the mother for any requests.  If the record is restricted (parents were not married at time of birth, father not named); please send/provide a photocopy of your driver’s license; since only those named on the certificate have a right to said document.

Death Certificates

Certified copies of death certificates can be obtained from the Town Clerk’s Office.  Death certificates are on file for decedents who were pronounced in the Town of Goshen, or who resided in the Town of Goshen at the time of death.  Persons who died out of state will not a have a record on file in the Town of Goshen.

The fee for a death certificate is $5 per certified copy (cash or check made out to the Town of Goshen), and can be obtained by mail or in person.  Please include with your request the decedent’s full name and date of death.

Marriage Certificates

Certified copies of marriage certificates can be obtained from the Town Clerk’s Office.  The record of marriage will only be here if the bride and groom filed for the marriage intention (application) in the town of Goshen.

The fee for a marriage certificate is $5 per certified copy (cash or check made out to the Town of Goshen), and can be obtained by mail or in person.  Please include with your request the bride’s maiden name and the groom’s name as well as the date of marriage.
